Tile broken. Crack tile with a hammer, dig out pieces and scrape adhesive from subfloor. Spread in new asphalt tile adhesive. After it becomes tacky, set in tile. If sub-floor is uneven, tile should be heated in an oven until it is pliable and then quickly set in place.
Holes. Replace tile as above.
Tile loose. This usually occurs when tiles are laid over a concrete floor that contains too much moisture. There's nothing to do but relay tiles with new adhesive after concrete has been allowed to dry completely.
